We will need the same paperwork that was given this year.
I will need a filled out copy of the ASA background form, which can be found here. We also need a photocopy of your driver's license.
We will need a copy of your child abuse clearance.
We will need a copy of your state police criminal record check which can be done here.

We will need a signed copy of the FBI disclosure form that can be found here.
*These 5 pieces of information can be completed ahead of time and mailed to our P.O. Box anytime. Otherwise, we will need them by February 1, 2017.
PLEASE NOTE RETURNING COACHES: Commonwealth Clearances are valid for FIVE years from the request date. If you coached from the beginning of the season and your clearances were submitted to ASA, I only need the ASA background form and driver's license copy from you. Any questions, please contact me.
